Background
Perform a literature review and create a summary of findings of similar projects to determine best practices for your project.
Discuss what has been done before that may be relevant and applicable to replicate at your community partner site.
With your community partner, determine which stakeholders should be contacted and perform key informant interviews. Develop a database of key informants and contacts (use your project folder accessed on your Data page).

My project will be a primarily qualitative look at why patients at Talbert House participate in primary care diabetes management. More specifically, I will accomplish this through shadowing in the clinic and standardized semi-structured interviews clinical staff and patients The shadowing will allow me to get a sense of the patients’ clinic experience as they interact with clinical staff and are processed through the clinic. The questions are designed to understand why Talbert House members do or do not choose to participate in the various interventions of the Primary Care division, specifically for diabetes. I plan to perform these interviews with patients over the phone and in person at the Primary Care clinic. In addition to these interviews, I will compare the A1c’s of patients who participate in more education/intensive treatment regimens vs those who do not.
This project was created as part of my service and advocacy elective. My partner, Talbert House, already was engaging in chronic disease prevention programs but during a meeting it was mentioned that some of their social and medical interventions are underutilized by Talbert House residents. I wanted to see if there were any generalizations that could be drawn about why people do or do not participate in these interventions.
