The purpose of this project is to create a partnership between the Environmental Health and Lead Clinic and People Working Cooperatively. We will pilot a system to connect children with Difficult to Treat or Severe Asthma from CCHMC. to a community agency, People Working Cooperatively, to carry out an environmental assessment and customized remediation in order to reduce in-home asthma triggers. This work builds upon previous quality improvement work already done at CCHMC to improve the medical management of children with asthma. In the process of developing this referral system, five (5) families with children with asthma will be successfully referred to People Working Cooperatively for home assessment and environmental remediation.
